They're wired up. A man in a van sees the replay virtually instantaneously and tells the fourth official/ref. Most likely when play is stopped e.g. as with the award of the penalty. Who is to judge which are the "incorrect facts" and which are the "correct ones?" One of the welcome innovations in recent times is the emergence of fact-checking organisations. You could try Fullfact or Infact if you want to understand facts as opposed to estimates/gestimates/lies. You may not like...
