Things continue to ramp up. We are now using non-ICU areas to care for ICU patients. Beds are not an issue yet but it probably will be.
More and more young people being admitted. The elderly or those with significant co-morbidities are being considered for ICU. The home teams along with ICU are making decisions on the appropriateness of admissions. This is nothing new and always happens when considering escalation above ward level care.
I was hoping McLaren F1 were going to create a new amazing ventilator, and watching the news that’s what someone whose doesn’t work in ICU might think. But it seems they have dusted off an old wispy flow system and given it an upgrade. They do work but waste a lot of 02. Something which may be an issue. Still they have done that in a hand Full of days so there is hope for more.
The public have been fantastic. Every day we are receiving gifts from businesses. Hand cream, food, coffee etc. Feels a bit early for all the love. It’s busy but it’s been busier. I hope the love hasn’t peaked too soon. I also hope the elderly wards, nursing homes, and other usual forgotten areas are getting gifts as boosts. As hard and stressful as it is in ICU it doesn’t compare to a day on a dementia ward.
We keep getting told we are 2/3 weeks away from the peak, yet Italy and Spain are not at their peak yet. Some of the consultants are predicting May will be the peak, but no one really knows.
Mentally it’s hard. I am seeing patients I don’t usually see. Fit people. People with only type 1 resp failure (low 02). Patients look very well until you do a proper assessment and arterial blood gas and discover they are profoundly hypoxic. This is confusing for all of us. Most people who normally require ICU look like they are on deaths door. Many of the covid patients look well but are not. The usual end of bed assessment (if they look unwell they are probably very unwell) doesn’t apply.
It’s weird that although it’s busy it isn’t as busy as the media might suggest (I don’t work in London). I sometimes have the thought that maybe it won’t get that bad. But then I realise that isn’t a helpful attitude. Prepare for the worst and hope for the best. They are not building hospitals in conference centres for no reason.

What’s different about the data is it looks at community deaths – people who died at home or in residential care whom doctors recorded on the death certificate as probably having Covid-19. There were 103 of these – 1% of all deaths.
So the 1% figure which most people have forecast as a rough estimate of the likely death toll from covid-19,however it might not even be that high as there's another titbit in the report.
In comparison, there are not enough tests to check how many people in the community are infected or how many deaths are linked to Covid-19.
The number of infections is certainly much higher than the official number of 22,000,indeed in Scotland alone the CMO estimated a couple of days ago the total of 65,000,and that's possibly an underestimate.
Clearly if you're only measuring the fatality rate by the hospitalised patients,then naturally it'll be much higher,as those patients are more seriously impacted by the illness,and obviously much more likely to sadly succumb to their symptoms.
Hopefully this puts the situation into perspective,yes of course we need to take this threat seriously,and follow the instructions,that should be plain,but these numbers should reassure some of the panic brigade,that's it's not the end of the world after all.

Sadiq Khan has written to London Premier League clubs asking that they allow the team doctors and others medical staff to help the NHS out. If it gets as bad up here and Burnley are asked I would like to think the club would allow our medical staff to help in the effort.

My son just gone for an operation in the RVI hospital in Newcastle scarily the surgeon has told the nurse to throw her clothes away afterwards and he is burning his. Within 4 weeks the Hospital will be shut and only open for covid. We aren’t hit hugely bad in the North East yet well not in the released data we aren’t but they reckon the Hospital is already filling up with them and obviously it is going to get worse. Get him get him home and sanitise everything I think is the order of the day.

The Government press conferences (at least from the ministers) have become about name dropping and bombarding with info (Maclaren, Lynx helicopters blah blah) whilst not actually saying anything.
The real question is why we are not testing at nearly the levels we should be, why why there are 44 molecular virology labs in the UK sitting idle but could have seen us at the level of testing Germany has had for weeks. Why they either were too incompetent or lied about getting in on the EU ventilator scheme.
Why we have been one of the last European nations to adopt every single kind of social distancing/lock down procedures.
The real question is why we are not testing at nearly the levels we should be, why why there are 44 molecular virology labs in the UK sitting idle but could have seen us at the level of testing Germany has had for weeks. Why they either were too incompetent or lied about getting in on the EU ventilator scheme.
Why we have been one of the last European nations to adopt every single kind of social distancing/lock down procedures.

Be careful with HCQ Frank it has massive retinal degenerative issues (its used as an arthritic revealing drug). Yes people will say it can help but the safest way is to stay safe and follow government guidelines.

In press conference today when was asked about low number of tests being done Gove said there was now a shortage of chemicals needed to make new testing kits.
Peston:
"I've just talked to the Chemical Industries Association, which represents the UK's very substantial chemicals industry. It has contacted its members, and they've said there is no shortage of the relevant reagents. So the Association has now been in touch with Gove's office to find out what he means, because it is stumped.
The Association also points out there was an industry chat with a business minister today, who made no attempt to find out if there was a supply problem for the vital ingredients of Covid19 testing kits.
So this question of why there aren't enough tests for the virus is an even bigger mystery. Also, if it turns out there is a shortage these manufacturers are more than happy to increase their production. But they need to be asked, which has not happened."
This from Gove who said we'd hit the 10k per day testing target at the weekend but turned out to be a lie.
Government is doing everything it can to not look like it's completely dropped the ball on this one while other countries successfully reduce the impact of the virus through wide scale testing.

The more this goes on, the more I think the Government handling is at best poor and at worse negligent.
We are running out of ICU drugs.

NY Governor: 'We underestimated this virus'
New York Governor Andrew Cuomo has called again for more supplies, a better national strategy to fight Covid-19 and for residents to stay at home.
"We underestimated this virus. It's more powerful, it's more dangerous than we expected," he emphasised.
The state, which remains the epicentre of the outbreak in the US, desperately needs more equipment and staff.
Cuomo called for a nationwide strategy where states helped mitigate the peak of the crisis together.
"Come help us in New York, get the equipment, get the training, get the experience, and then let's all go help the next place," Cuomo said.
"That would be a smart national way of doing this and showing that unity."
He criticised the "inefficient" US process for ordering ventilators, describing it as "being on eBay with 50 other states bidding on a ventilator". He added once the federal government got involved and was bidding, prices continued to go up.
The governor also addressed his brother CNN anchor Chris Cuomo's Covid-19 diagnosis.
"He’s a really sweet, beautiful guy, and he’s my best friend," Cuomo said.
"There’s a lesson in this. He’s an essential worker...so he’s been out there. If you go out there, the chance that you get infected is very high."

New York has now reached 1,550 deaths from 1,218 yesterday
Experts are predicting the peak will be in the next 14 to 30 days
The state has 75,000 hospital beds in total as hospitals have added to their capacity, but there is an anticipated maximum need of 140,000 at the apex
The state could need anywhere from 20,000 to 40,000 ventilators depending on the severity of the apex; despite ordering 17,000 from China, Cuomo is expecting 2,500 to arrive due to high global demand
Cuomo claims the state is leading the nation in developing new medications and therapies

Reading various reports it's clearly obvious there is absolutely no shortage of the chemicals needed and it's just a bare faced lie on the part of Gove. That's shocking but a 12 year old died in Belgium,a 14 year old in Portugal,and a 16 year old in France,and to my knowledge they weren't suffering from any underlying health conditions,now these are isolated cases,but they do highlight that youth alone doesn't automatically mean you're safe from harm.
Hopefully this tragedy will reinforce the message to stay at home,as Jenny Harries outlined this afternoon.

Keep looking at the statistics and I start to believe the conspiracy theories that either the Chinese started this or are completely out with the numbers. It went unchecked in China for a longer period than any other country , so how on earth did they stop it in its tracks at 80k cases.

They can lock down entire cities, build hospitals in a week, track everyone at all times, retool entire production lines, mobilize millions of medics.
And the public is so used to this they neither complain and happily submit to the invasions of privacy and curtailment of freedoms.
So I'm not really surprised they were able to halt it, they smothered it very effectively at the price of freedom.
