Ventilator dilema
Suppose in the coronavirus crisis there is one ventilator but 2 patients needing it. One is a smoker the other a non-smoker. Who should have priority?

Would also depend on age and other underlying conditions.

Id say smoker cos with all the sh*t in their lungs their need will probably be greater

DOes it matter if the smoker is a 30 year old mother of 4 and the non smoker is a 95 year old ex Nazi?

You'd treat it like a transplant and say that the one treating their organs better deserves it.
You wouldn't donate a set of lungs to a smoker if the choice is between the two.

That’s exactly how they’d assess it, basically it’d be down to who they think could live the longest, no different to lots of operations & treatments, elderly patients with cancer it’s not uncommon to advise against chemotherapy as they know the chemotherapy would kill them there’d rather manage the cancer differently more towards palliative care.

Italy are doing it two ways.
80 yrs + and they're not going to be treated etc.
Anyone under 80, if they've got various health issues already that would likely prevent recovery and they're also being denied treatment.

Unfortunately we've given livers to alcoholics and then they've carried on drinking excessively

Tough call! i'm just glad i don't have to make that decision thankfully.

Alcoholism is not a disease of the liver but the mind.Alcoholics drink to excess, if they could control their drinking they wouldn't be alcoholics.
You have to be abstinent for 6 months to get on the transplant list.
