I find it infuriating. It really isn't difficult to get a persons name correct especially when it is a name as simple as Wood. It's common courtesy IMO.

It's likely because they are thick, disrespectful enough to not get someones name right, or dyslexic

Generally it's going to be the first 2 isn't it?
