Help required - Commemorative medal 1999/2000 season

I've come into the possession of a boxed gold medal, 1 and 1/2inches in diameter and engraved as follows - on one side there is the BFC club crest in the middle, above that Burnley Football Club and under the crest Division One Promotion 2000. On the other side it says - On behalf of the Chairman, Directors, Management, Players and all Staff. Thank you for your support.

Any ideas as to when these were given out and who to?
